Beneficial Owner
The submitted Personal Declaration Form (PDF) to Malta Gaming Authority (MGA) must contain every person that holds 10% or more equity, voting, or control rights in a licensee or license applicant. The PDF may also require any other individuals who are investing funds into the licensee or license applicant.
Beneficial owners are also required to submit:
- Source of Wealth Declaration Form – Must be submitted by ALL ultimate beneficial owners (UBOs) and/or third parties that are investing funds into a licensee or license applicant. This form must be supported by detailed and relevant evidence/proof to account for the funds invested in the operations.
- Statement of Affairs Form – Must be fulfilled by all UBOs and/or third parties that are investing funds into a licensee or license applicant. The MGA may request for additional information required during the application of the license, such as evidence to support this form.
How to Apply
The following forms below must be filled in by unique individuals in the Personal timeline within the licensee portal. In the Personal timeline, the ‘Change in Personal Details’ application, specifically in “Step 3 Involvement Type”, is used to request for relevant involvement with the company.
Upon the submission of the ‘Change in Personal Details’ application, the Request for Involvement will be shown in the company timeline or draft application, and persons with the necessary access to the company will approve or decline the request. The approval or rejection of the roles would signify the finalisation of the Company’s responsibilities, thus requiring the MGA to proceed with the final steps of the process.
